What Happens During Your First Therapy Session?
Starting your first therapy session can feel both hopeful and unfamiliar. Many people wonder what will happen, what they should say, or whether they need to prepare in advance. These thoughts are completely normal. Whether you attend in person in London or connect online, a first therapy session is simply an opportunity to begin a conversation in a safe, confidential and non-judgemental environment where you can explore whatever has brought you to therapy.
Whether you are seeking support for anxiety, stress, low mood, relationship difficulties, trauma or simply feeling overwhelmed, the aim of the first therapy session is to understand your needs and discuss how therapy may help.
What Happens in Your First Therapy Session?
During your first therapy session, we begin by talking about what has brought you to therapy and what you hope to gain from it. There is no expectation that you have everything worked out before you arrive.
You might choose to discuss:
- What has been happening recently
- Difficult emotions or thoughts
- Relationships
- Work or family pressures
- Previous experiences of therapy
- Your goals for counselling
Some people speak freely from the beginning, while others need more time. Both approaches are completely normal.
Do You Need to Prepare for Your First Therapy Session?
Most people do not need to prepare anything.
Some clients like to think about:
- Why they decided to seek therapy
- What they would like to change
- Questions they would like to ask
However, many people simply attend their first therapy session without preparation and find that the conversation develops naturally.
There is no right or wrong way to begin therapy.
Will I Have to Talk About Everything?
No.
One of the biggest misconceptions about a first therapy session is that you must immediately discuss your deepest experiences.
Therapy works at your pace.
Building trust takes time, and you are always free to decide what you feel comfortable talking about.
The aim is not to pressure you but to create a space where you feel safe enough to speak openly when you are ready.
How Long Does the First Therapy Session Last?
Most counselling sessions last approximately 50 minutes.
Towards the end of your first therapy session, we usually spend a few minutes reflecting on how the session felt and discussing possible next steps.
If you decide to continue, we can agree on a frequency of sessions that suits your individual needs.
What If I Feel Nervous?
Feeling nervous before your first therapy session is extremely common.
Many clients tell me afterwards that they were worried beforehand but felt relieved once the conversation had started.
You do not need to impress your therapist or say the “right” things.
Therapy is about understanding your experiences—not judging them.
7 Things to Expect During Your First Therapy Session
1. You will be listened to without judgement
Your therapist’s role is to understand your experiences, not to criticise or tell you what to do. Many people say that simply having a space where they feel genuinely heard is one of the most valuable parts of therapy.
2. We will work at your pace
You are never expected to talk about anything before you feel ready. If there are subjects you would prefer not to discuss during your first therapy session, that is completely acceptable.
3. You can ask questions
Your first appointment is also an opportunity to ask questions about how therapy works, confidentiality, session frequency, or anything else that is important to you.
4. We will discuss your goals
Some people arrive with very clear goals, while others simply know that something needs to change. Together, we will begin exploring what you would like to achieve through therapy.
5. Therapy is collaborative
Therapy is not something that is “done to you.” It is a collaborative process where we work together to understand your experiences, develop insight and support positive change.
6. There is no obligation to continue
At the end of your first therapy session, you can decide whether you feel comfortable continuing. Finding the right therapist is important, and it is perfectly acceptable to take time before making a decision.
7. Every therapeutic journey is unique
No two clients are the same. Therapy is tailored to your individual needs, experiences and goals rather than following a fixed programme.
Frequently Asked Questions
Is it normal to feel nervous before your first therapy session?
Yes. Feeling anxious or uncertain before your first appointment is completely normal. Most people find that these feelings begin to settle once the conversation starts.
Do I need to bring anything?
No. You do not need to prepare anything specific. Simply come as you are. If there is anything you would like to discuss or ask, you are welcome to make a few notes beforehand, but this is entirely optional.
Will everything I say remain confidential?
Confidentiality is a fundamental part of therapy. There are a small number of legal and ethical exceptions, which your therapist will explain during your first session.
How often should I attend therapy?
This depends on your individual needs. Many people attend weekly sessions, while others prefer fortnightly appointments. Whether you’re based in London or joining online, we can discuss what feels most appropriate for you.
